Primary Purpose of Position: As a Patient-Centered Medical Home (PCMH), we are committed to providing accessible, high-quality, team-based care. This position is designed for an on-site clinician who functions as the "physical anchor" for a shared patient panel, working in a dyad leadership model with a remote Primary Care Clinician. Reporting to the Site Medical Director, the Access Clinician serves as the on-site clinical lead for a designated patient panel. Working in a formal dyad partnership with a Remote PCP, this role focuses on providing in-person clinical exams, urgent care interventions, and procedural support that cannot be performed via telehealth, ensuring seamless continuity of care.
