Founded in 1987 through the merger of Health Associates of Provincetown (established in 1972) and the AIM Medical Center in Wellfleet (established in 1966). Primary Purpose of Position: Reporting to the Site Medical Director, the Primary Care Clinician serves as a clinical leader within an interdisciplinary care team and provides comprehensive, continuous care to a panel of patients. The clinician collaborates with team members to coordinate care, promote health, manage chronic disease, and improve patient outcomes in alignment with OCHS’s mission and team-based care model.
