About The Position

The Program Manager of Oak Hill’s Supported Living Arrangement (SLA) and Community Companion Home (CCH) programs works 40 hours/week; schedule Monday-Friday 8a-4p with flexibility required and provides leadership to staff and host families, ensures regulatory adherence, supports individuals in achieving meaningful community integration, and serves as a primary liaison with DDS and other state agencies

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field
  • 3 years experience in operations and supervision in a developmental disability services environment.
  • Familiarity with Connecticut DDS regulations, including licensing standards for CCH compliance.
  • Strong leadership, communication, and organizational skills.
  • Valid driver’s license.

Responsibilities

  • Oversee individualized home supports and care coordination for residents living independently or semi-independently.
  • Ensure compliance with DDS standards for Supportive Housing and Individualized Home Supports.
  • Supervise direct support staff and ensure proper training, documentation, and service delivery.
  • Develop person-centered plans and monitor behavioral support strategies.
  • Manage the licensing, training, and oversight of CCH host families in accordance with DDS protocols.
  • Monitor home environments for safety, inclusion, and compliance with DDS licensing and care standards.
  • Maintain documentation and reporting in accordance with DSS, Social Security, DDS and Medicaid HCBS requirements.
  • Ensure all services meet the standards outlined in the Connecticut Statewide HCBS Transition Plan.
  • Provides direct support as needed to meet programmatic need.
  • Becomes familiar with the individuals in their assigned location(s); participates in defining goals and objectives for assigned location(s); plans, implements and evaluates programs, services and activities in collaboration with the Program Director or Program Coordinator and other members of the Planning and Support Team (PST); ensures services meet individuals’ needs; coordinates and participates in all individual related meetings; ensures that program operations comply with state and federal regulations, Oak Hill policies and procedures and with health and safety regulations.
  • Ensures that the locations’ physical environment meets required regulations, including a clean, safe, and homelike atmosphere; supports the process for all contractual agreements, including Department of Developmental Disabilities (DDS), Department of Children and Families (DCF), Intermediate Care Facilities (ICF) and Oak Hill policies and procedures.
