This position is scheduled to work 4 to 5 days per week depending on preference. GENERAL POSITION DESCRIPTION: The Public Health Educator & Navigator will promote, maintain, and improve individual and community health and well-being by designing and implementing public health education and outreach programs with a public health focus on primary prevention (including chronic disease prevention, tobacco education, etc.) and community behavioral health service navigation, promotion and support. This role will actively engage the community, lead public awareness efforts, stigma reduction and provide essential navigation and referral coordination to connect at-risk and underserved populations. The incumbent will collect and analyze data to identify community needs prior to planning, implementing, monitoring and evaluating programs to encourage health lifestyles, policies and environments. Please note that this is a grant-funded position. Employment is predicated on the continued availability of grant funds. In the event that funding is reduced or discontinued, the duration of the appointment may be adjusted accordingly. The primary location of employment for this position is the designated Delta County office with the exception of required duties that are to be performed off site as necessary. The employee is required to perform the majority (generally, at least 95%) of assigned duties from this designated office location to allow for client, customer and coworker access and collaboration, unless prior written approval for an alternative arrangement, such as temporary remote work, has been granted by the employee's direct supervisor and/or department head and maintained in the employee personnel file.
