About The Position

This position is scheduled to work 4 to 5 days per week depending on preference. GENERAL POSITION DESCRIPTION: The Public Health Educator & Navigator will promote, maintain, and improve individual and community health and well-being by designing and implementing public health education and outreach programs with a public health focus on primary prevention (including chronic disease prevention, tobacco education, etc.) and community behavioral health service navigation, promotion and support. This role will actively engage the community, lead public awareness efforts, stigma reduction and provide essential navigation and referral coordination to connect at-risk and underserved populations. The incumbent will collect and analyze data to identify community needs prior to planning, implementing, monitoring and evaluating programs to encourage health lifestyles, policies and environments. Please note that this is a grant-funded position. Employment is predicated on the continued availability of grant funds. In the event that funding is reduced or discontinued, the duration of the appointment may be adjusted accordingly. The primary location of employment for this position is the designated Delta County office with the exception of required duties that are to be performed off site as necessary. The employee is required to perform the majority (generally, at least 95%) of assigned duties from this designated office location to allow for client, customer and coworker access and collaboration, unless prior written approval for an alternative arrangement, such as temporary remote work, has been granted by the employee's direct supervisor and/or department head and maintained in the employee personnel file.

Requirements

A high school diploma or GED is required.
Must have at least three (3) years of experience in any of the following: behavioral health, public health, human services, adult education, health education, community outreach, or case management.
Clear, professional written and verbal communication skills are required.
Current, valid and unrestricted Colorado Driver’s License is required.

Nice To Haves

An associate's or bachelor’s degree in public health, behavioral health, social work, communications, adult education or a related field is preferred.
basic understanding of behavioral health and community resources
bilingual (e.g., Spanish/English) proficiency is strongly preferred
experience with community outreach and public engagement
experience creating educational or promotional materials.

Responsibilities

Plan, promote, implement, and evaluate public health education and prevention programming to targeted populations.
Develop and deliver public health and behavioral health outreach, presentations, and community events. Markets health education efforts and programs to the general public and local businesses.
Increase awareness and reduce stigma surrounding behavioral health through public messaging, education, and engagement activities.
Provide navigation support for at-risk and underserved populations, including teens, postpartum women, isolated older adults, and rural residents.
Reduce barriers and increase access to public health and behavioral health services through connection, referral coordination, and follow-up.
Collect and analyze data on screenings, referrals, service utilization, and program outcomes.
Prepare reports to evaluate program impact and inform future education and outreach activities.
Contribute to grant writing, reporting, and sustainability planning efforts for behavioral health initiatives.
Engage in appropriate training to ensure program competency.
Assure program compliance with contract requirements.
Participate in data collection, analysis and reporting.
Develop and maintain a timeline for program activities.
Participate in health program monthly, quarterly and yearly meetings.
Help organize and attend events promoting program activities.
Provide services for prevention and cessation of tobacco use.
Order and stock needed supplies for all health promotion programs.
As required, promote the development of a wellness/tobacco and chronic disease prevention coalition.
As required, plan, organize and conduct a needs assessment for tobacco control issues.
Research evidence-based tobacco control best practices for introduction to vulnerable communities.
Identify the diversity of community interests and concerns that surround the use of tobacco, and prioritize needs based upon findings.
Actively participate in professional and related organizational partnerships.
Interact with the public in a friendly, tactful manner to resolve problems.
Receive public communications and respond or direct to the appropriate area.
Demonstrate core competencies within the Incident Command System structure as appropriate to this position’s functional role in a public health emergency response event. (Completion of NIMS-700, ICS-100, ICS-200)
Should the Board of County Commissioners or another authorized representative issue a local disaster declaration in Delta County, all Delta County employees may be called upon to serve as disaster service workers.
Perform other similar and related duties within the department as assigned.
