As a Quality Assurance Caseworker for the Office of Aging and Disability Services, you will: Perform professional social work for eligible adults with developmental disabilities, including state case management. Participate in care planning which includes plan writing and extensive documentation. Facilitate meetings with provider agencies to review quality assurance benchmarks. Communicate with provider agencies on a quarterly basis reviewing data related to reportable event compliance. Community outreach and education when applicable. Your workload will include: Advocating for and coordinating provision of services including reviewing, analyzing, and documenting person-centered plans. Maintaining accurate case records of assessments, activities, and plans; writing summaries, reports, letters, and memos. Collaborating with Adult Protective Services and Crisis Prevention/Intervention Services when necessary to stabilize and secure emergency needs. Analyzing data and preparing reports for quality outcomes. Utilizing the QA Sharepoint database keeping it updated with required documentation. Presenting data reports to assigned agencies on a quarterly basis.
