About The Position

CVP is seeking Registered Nurses to join our team in providing our Nation's Veterans with long-term care services at the Cincinnati VA Medical Center-Fort Thomas. The RN will utilize their broad based clinical knowledge to effectively assess, implement health promotion and prevention interventions, manage acute and chronic illness states, assist patient/residents in the attainment of optimal levels of functioning rehabilitation, and evaluate patient/resident response to actual or potential health problems. We have full time, part-time and PRN contract opportunities available.

Requirements

  • Ability to pass a government background investigation (US Citizenship required)
  • Bachelor's Degree in Nursing or a Master's Degree in Nursing required
  • Current, full, active, and unrestricted RN license in any State, Territory, or Commonwealth of the United States or District of Columbia
  • Basic Life Support (BLS) certification from the American Heart Association
  • 2+ years providing Geriatric, Community Living or Long Term Care
  • Technical proficiency including strong computer skills and ability to learn VA electronic medical record systems

Responsibilities

  • Utilize the nursing process in managing, developing, implementing, and evaluating each resident's individual plan of care with the interdisciplinary team to attain specific clinical outcomes
  • Provide a full range of nursing care to residents with a variety of physical and/or behavioral problems
  • Demonstrate leadership in delivering and improving holistic care through collaborative strategies with others
  • Maintain patient/resident confidentiality and a safe patient/resident care environment
  • Assess and act on significant changes in the patient's/resident's health condition
  • Collect and document pertinent data into the electronic patient/resident medical record using designated templates
  • Complete nursing assessments within required time frames and in accordance with policy and procedure
  • Meet the health care needs of patients/residents by assessing and assisting them to meet expressed needs
  • Prioritize and complete tasks within the assigned shift
  • Update patient/resident care plans and notify assigned physician of any changes in condition
  • Practice the VA core values of Integrity, Commitment, Advocacy, Respect, and Excellence (ICARE)
  • Document patient progress and chart the patient's response to medications and treatments
  • Perform wound care including dressing changes (sterile and non-sterile) and obtaining wound cultures as necessary
  • Provide care for tracheostomies, central lines, PICC lines, feeding tubes, and catheters as needed
  • Perform fall risk assessments and skin assessments (Braden Scale) to identify and prevent injuries
  • Complete required documentation within established timeframes
  • Participate in quality improvement activities
