Join Essentia’s accomplished team as a Registered Nurse on our Anticoagulation Management Team. Education Qualifications: BSN or ADN degree from an accredited school or college of nursing. What You'll Do in This Role: Deliver Comprehensive Care: Provide nursing services in primary and specialty outpatient facilities, non-acute community settings, and through telehealth encounters, ensuring accessible care for all patients. Exercise Professional Autonomy: Take responsibility for your nursing practice by making informed decisions and promoting health, recognizing that quality care is a universal right. Collaborate with Healthcare Teams: Work closely with patients and a multidisciplinary team to assess concerns, identify issues, and integrate both subjective and objective data. Additional Responsibilities for Anticoagulation RN: Monitor and manage anticoagulation therapy, including dosing adjustments based on lab results such as INR, to ensure patient safety and therapeutic effectiveness. Educate patients and caregivers on anticoagulation therapy, potential side effects, dietary considerations, and adherence to medication regimens. Perform patient risk assessments to identify bleeding or thrombotic risks and adjust care plans accordingly. Coordinate with physicians and pharmacists to optimize anticoagulation management and troubleshoot complications. Maintain accurate documentation of patient assessments, therapy changes, and communication to support continuity of care. This individual will work 3 days onsite at our Hermantown Clinic and 1 day remotely. This position will work the following: 64 hours per pay period No Weekends No Holidays Day Shift: 8:00AM - 4:30PM
