About The Position

Affirmed Home Care is seeking experienced and compassionate Registered Nurses (RNs) to provide dedicated one-on-one care for private clients in Katonah and Southampton, New York. You’ll work closely with clients and their families while supported by a highly responsive and professional clinical team.

Requirements

Active New York State Registered Nurse (RN) License
1–2+ years of clinical nursing experience preferred (home care, private duty, med-surg, or geriatrics)
Strong clinical judgment and ability to work independently in a home care setting
Excellent communication and patient-care skills
BLS / CPR Certification (e.g., from the American Heart Association)
Reliable transportation preferred
Experience with Electronic Health Records (EHR) systems is a plus

Responsibilities

Provide one-on-one skilled nursing care in the client’s home
Perform comprehensive assessments and develop individualized care plans
Administer medications and treatments according to physician orders
Monitor patient condition and respond promptly to changes in health status
Deliver specialized care such as wound care, IV therapy, and catheter management
Educate patients and families on disease management and home safety
Maintain accurate clinical documentation in compliance with regulatory standards
Collaborate with physicians, care coordinators, and family members to ensure optimal care
