In this critical role, you will shape the planning and execution of in-person care delivery across various markets. Your responsibilities will include establishing operational foundations, ensuring that care teams have the tools and support necessary to deliver high-quality encounters, and driving the effectiveness of care models that positively impact patient experiences and health outcomes. Requirements

  • 5+ years of experience in healthcare operations or care delivery programs.
  • Proven experience in building or improving field-based operational models.
  • Ability to translate health plan requirements into repeatable workflows.
  • Experience partnering with clinical teams and cross-functional stakeholders.
  • Prior experience in managing and developing operational teams.
  • Strong operational judgment and problem-solving skills.
  • Comfort working with data to drive performance assessments.
  • Experience creating and maintaining operational documentation.
  • Familiarity with scheduling systems and project management tools.
  • Excellent communication skills to influence and navigate ambiguity.

Responsibilities

  • Own the design, execution, and ongoing improvement of in-person care delivery operations.
  • Build scalable workflows that support field-based visits, including care package delivery and care plan check-ins.
  • Translate health plan and payer requirements into clear operational processes for care teams.
  • Establish standards and quality checks for in-person visit bookings.
  • Serve as the operational point of contact for in-person care delivery.
  • Develop and maintain tools, documentation, and SOPs for planning field visits.
  • Oversee and continuously improve visit scheduling, tracking, and documentation workflows.
  • Manage and develop a team providing support for in-person encounters.
  • Partner with clinical and operations leaders to align in-person encounters with care plans.
  • Own field operations metrics and utilize data for process improvements.
