About The Position

The Harbor Haven Supportive Housing Program is an innovative supportive housing program based in San Francisco’s Bayview/Hunters Point community. Funded through the U.S. Department for Housing and Urban Development (HUD); Harbor Haven is a community-based housing program. The goal of Harbor Haven Supportive Housing Program is to integrate permanent supportive housing with support services that will assist chronically homeless and disabled individuals with meeting their basic needs, establishing a sense of self mastery, and living a life of housing stability. Program staff work directly with participants to access and maintain support services by working on three fronts: 1) Providing permanent supportive housing for the chronically homeless in a community living environment; 2) Working with various agencies to address and support participants’ needs in regards to mental/physical health and/or substance abuse; 3) Work with multiple agencies to assist program participants in accessing employment/volunteer training programs to increase the likelihood of economic improvement. Case Managers serve as liaisons and advocates that provide supportive administrative coordination between program participants, collaborating agencies and other accessible resources. Facility Operations & Resident Care - Support the day-to-day operation of the facility to ensure a safe, well-maintained environment where residents receive appropriate and timely services. Resident Support & Tenancy Stabilization - Assist residents in maintaining stable housing by addressing barriers to tenancy, promoting independence, and supporting overall quality of life. Programming & Community Engagement - Plan, coordinate, and facilitate resident events, workshops, and activities that foster community connection, engagement, and personal growth. Crisis Intervention & Case Management - Provide crisis intervention, ongoing case management, and follow-up services to support residents during emergencies or periods of instability. Documentation & Reporting - Maintain accurate and up-to-date records of resident services, billing documentation, progress notes, and required reports in compliance with organizational and funding requirements. Service Planning & Monitoring - Develop, implement, and regularly review individualized treatment or service plans, documenting progress and adjusting goals as needed. Community Referrals & Resource Coordination - Connect residents to internal and external community resources, including employment services, healthcare, legal assistance, education, and peer-support programs. Ongoing Communication & Assessment - Maintain consistent communication with residents to assess wellbeing, identify emerging needs, and modify service plans accordingly.

Requirements

  • Minimum of 1–2 years of experience in case management, supportive housing, social services, or a related field.
  • Knowledge of tenancy stabilization, crisis intervention, and trauma-informed care practices.
  • Experience working with diverse populations, including individuals facing housing instability, mental health challenges, or other barriers to self-sufficiency.
  • Ability to develop, implement, and document individualized service or treatment plans.
  • Strong organizational and documentation skills, with experience maintaining accurate records and completing required reports.
  • Excellent communication and interpersonal skills, with the ability to build trust and maintain professional boundaries.
  • Familiarity with community resources related to healthcare, employment, legal services, and social supports.
  • Proficiency with basic computer systems and case management or electronic record-keeping platforms.
  • Ability to work independently and collaboratively in a team-based environment.
  • Ability to travel locally as required for job duties.
  • Valid California driver’s license and reliable transportation required.
  • Ability to pass background checks and meet agency, state, or funding-source requirements.

Nice To Haves

  • CPR/First Aid certification and/or crisis intervention training preferred (or willingness to obtain upon hire).
