The primary responsibility of the Residential Care Manager is to provide comprehensive case management services to at-risk youth residing in residential group care settings. The Residential Care Manager administers assessments, coordinates treatment teams, develops and monitors progress toward individualized care plans, facilitates therapeutic and enrichment programming, delivers evidence-informed and trauma-responsive interventions, and ensures accurate documentation and data management. The position operates under the supervision of the Director of Residential Care and Compliance. This role has primary responsibility for service coordination, care planning, documentation oversight, and collaboration with internal and external partners. This position reports to the Director of Residential Care and Compliance Scope: Residential Care Manager services include but are not limited to assessment, service planning, monitoring, coordination, and supervision of family visits and family engagement, group activities, community referrals, and educational and enrichment planning, coordination, and support. Services are based on youth needs and residential staff needs and are delivered in accordance with agency standards, licensing requirements, and program expectations. This position is also expected to document monthly progress notes in Landmarks for Families' data management system and share all required information regarding youth with the South Carolina Department of Social Services (SCDSS).
