About The Position

Reporting to the Agency Director, the Residential Coordinator contributes to the Stellar Care team effort by supporting and communicating the Stellar Care & Services mission, vision, strategic plan, goals, and management decisions. He/she ensures Stellar Care policies and all HCPF rules and regulations for the State of Colorado are followed. This role provides both administrative support and direct support coverage for the Residential and PCA programs.

  • This position is designated as an unlimited driving position and requires operating a motor vehicle as part of Stellar Care duties, including transporting persons served. A current, valid U.S. driver’s license and acceptable driving status are required. Employee is responsible for maintaining personal automobile insurance.
  • One year of administrative or coordination experience in the human services field; or equivalent experience.

Responsibilities

  • Coordinates administrative and operational support for the Residential Department.
  • Assists Program Directors with uploading, organizing, and maintaining documentation within the OnTarget system.
  • Provides case coverage and support when Program Directors are out of the office or during high‑volume periods.
  • Assists with case compliance tasks including audits, record reviews, corrective actions, and follow‑up.
  • Supports the Director and Department Senior with administrative projects, reporting, scheduling, and department operations.
  • Assists with enrollments, transitions, internal case changes, and service coordination as requested.
  • Maintains accurate participant records and documentation.
  • Helps ensure staffing coverage and continuity of services across residential homes.
  • Participates in a rotating on‑call schedule, including evenings and weekends as assigned.
  • May carry a small caseload or provide temporary oversight for assigned individuals as needed.
  • Provides direct support to individuals when necessary to ensure health, safety, and service continuity.
  • Collaborates with families, guardians, providers, and interdisciplinary teams.
  • Represents the Program Director or Director at internal and external meetings when requested.
  • Consistently exercises independent judgment, decision‑making, and problem solving.
  • Performs other related duties as assigned.
