About The Position

JOB SUMMARY The Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Team Lead/Manager is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. DUTIES & RESPONSIBILITIES The Clinical Manager should be available during all office hours to manage the day to day flow of clinical needs in the agency. Responsible for coordinating and conducting, as needed, the education and training of new clinical staff – with the exception of therapy staff - and oversees their ongoing orientation plan. Provides direct oversight of all nurses, aides, social workers, and chaplains performing field work. Responsible for assuring the development, implementation, and updates of the individualized plan of care, which would entail communication with all physicians involved in the plan of care and integration of orders, including medication orders, from all physicians involved in the plan of care. Responsible for ensuring that the On-Call schedule is complete and that on-call services are available when needed. Conducts clinical review of new patient referrals and coordinates with intake team to accept those referrals that the agency is capable of providing services for. Consults with intake for clinical oversight for appropriateness of case, including home visits, to determine home health care needs. Ensures that the appropriate home care personnel are assigned to the case as needed. Conferences with Attending Physician regarding any questions about an individual's eligibility for services. Reviews patient's medical diagnosis, procedures, medications, and clinical course. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care. Attends and runs and documents case conference meetings with organization personnel to facilitate coordination of care. Participates in quarterly record reviews and communicates findings and recommendations to Director of Clinical Services and appropriate organization personnel. Assists in the screening and interviewing process of new organization personnel. Makes recommendations for employment of individuals. In conjunction with Director of Clinical Services plans, implements and evaluates in-service and continuing education programs. Complies with accepted professional standards and principles. Participates actively in quality assessment performance improvement program. Ensures that infection control standards are implemented and followed in accordance with state and federal guidelines. Performs other duties and activities as delegated by the Director of Clinical Services and/or Executive Director.

Requirements

  • Must have graduated from an approved school of nursing and be licensed in the state where practicing
  • Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred
  • Maintains a current CPR certification
  • The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job
  • Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile
  • Self-directed and able to work with minimal supervision
  • Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist
  • Shows ability to organize and prioritize workload independently
  • Responsible for supervising the clinical team
  • Prolonged or considerable walking or standing
  • Able to lift, position, or transfer patients
  • Able to lift supplies and equipment
  • Considerable reaching, stooping, bending, kneeling, or crouching
  • Visual acuity and hearing to perform required nursing skills
