About The Position
Centurion is proud to be the provider of comprehensive healthcare services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Full-Time RN Discharge Planner to join our team at Maryland Correctional Institution located in Hagerstown, Maryland. The RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.
Requirements
- Current Registered Nurse in the state of Maryland or the ability to obtain license required
- Current BLS and CPR certifications required
- On-year experience as a Discharge Planner or Case Manager required
- Ability to obtain a security clearance to include drug screen and criminal background check
Nice To Haves
- Experience in corrections or an acute setting preferred
