About The Position

A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing/Branch Director/Clinical Manager.

Requirements

  • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
  • Two (2) years’ nursing experience, preferred.
  • Acceptance of philosophy and goals of this Agency.
  • Ability to exercise initiative and independent judgment.

Responsibilities

  • Provides services in accordance with the plan of care.
  • Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
  • Initiates the plan of care and necessary revisions.
  • Provides those services requiring substantial specialized nursing skills.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • The registered nurse must be the case manager in all cases involving nursing or both nursing and therapy care.
  • The registered nurse provides services within the scope of practice authorized by the license issued by the state of Florida for a registered nurse.
  • Registered nurses are responsible for the clinical record for each patient receiving nursing care as well as assuring that progress reports are made to the physician or practitioner when there is a change in patient condition or there are deviations from the plan of care.
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Coordinates services.
  • Informs personnel of changes in the condition and needs of the patient.
  • Counsels the patient and family/significant others in meeting nursing and related needs.
  • Participates in and presents in-service programs.
  • Understands and adheres to established Agency policies and procedures.
  • Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
  • Determines the amount and type of nursing needed by each individual patient.
  • Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
  • Supervises LPNs, Certified Nursing Assistants, Home Health Aides/HHA’s. and provides education as needed.
  • Conducts Annual In Home Supervisory visits on LPNs, Certified Nursing Assistants and Home Health Aides.
  • In Florida, A home health agency, providing skilled or nonskilled care, must have an RN provide supervisory visits of unlicensed personnel. The agency must obtain written consent from the patient or legal guardian to send an RN into the home to conduct supervisory visits. If the home health agency provides skilled care, the RN may delegate tasks in addition to personal care tasks, such as medication administration, if the home health aide or certified nursing assistant is qualified to perform these tasks as outlined in AHCA Florida Standards 59A-8.0097, 59A-8.0216, and 59A-8.0219 subject to Governing Body Approval. The registered nurse maintains full responsibility for tasks assigned to the aide A registered nurse may delegate tasks to a home health aide or certified nursing assistant (CNA) provided the following criteria are met: o The home health agency is licensed to provide skilled care. » The home health aide or CNA has met the training and validation requirements. o The registered nurse delegating the task determines that the home health aide or CNA is competent to perform the task. o The registered nurse shall follow the delegation process of the agency developed in accordance with Florida law (Rule 64B9-14.002, F.A.C.). A registered nurse delegating a task to a home health aide or CNA shall document the following: Name of the patient and task being performed. The expected or desired outcome. The time frame for the delegation. Verification of the home health aides or CNA’s understanding of the task. Periodic inspection and assessment of the home health aide or CNA’s performance of the task. o The registered nurse maintains accountability for the patient. The registered nurse may not delegate tasks that: o Are not within the delegating or supervising nurse’s scope of practice. o Require the use of the nursing process including assessment, diagnosis, planning, implementation, and evaluation. o Require the knowledge, judgment, or skills of a licensed health care professional. The home health agency must obtain written, dated consent of the patient, or the patient’s guardian, legal representative, or designated health care surrogate, to accept performance of delegated tasks, achc.org | Florida Survey Requirements including medication administration, by a home health aide or CNA, prior to services being provided. The consent must be maintained in the patient’s medical record and must contain the following: o Name of the patient. o The task(s) being performed. o The time frame for the delegation of the task(s). o The nature of the supervision required. » Signature of the patient, or the patient’s guardian, legal representative, or designated health care surrogate.   • Conducts patient care conferences on patients assigned to his/her care.
  • Participates in peer review and Quality Assurance and Performance Improvement as assigned.
  • Gives total patient care as needed.
  • Takes on-call duty nights, weekends and holidays, as assigned.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
  • Appropriately utilizes ICD-10 codes.
