Requirements
- Active RN license in Indiana (required)
- Long-Term Care experience (required)
- Strong attention to detail and organizational skills
- Clinical confidence and critical thinking ability
- Team-oriented mindset with strong communication skills
Responsibilities
- Lead and complete MDS assessments and care plans in compliance with federal and Medicare guidelines
- Coordinate Medicare A (PPS) scheduling and maintain accurate timelines
- Ensure documentation is accurate, timely, and audit-ready
- Collaborate with nursing, therapy, and interdisciplinary teams
- Identify and respond to significant changes in resident conditions
- Guide and support staff on RAI processes and documentation best practices
- Oversee and maintain the Restorative Nursing Program
- Play a key role in driving quality care and reimbursement accuracy
