We are a fast-paced, expanding Community Health Center seeking a Triage Registered Nurse (RN) to join our team and help improve the health and wellness of our neighbors. The Triage RN plays a key role in supporting patient care through phone triage, care coordination, and clinical follow-up. This position requires strong clinical judgment, organization, and the ability to thrive in a fast-paced environment while working both independently and collaboratively with the care team. Requirements
- Current PA (or compact) RN license
- Current CPR/BLS certification
- Strong critical thinking, multitasking, and organizational skills
- Ability to work independently and as part of a team
Nice To Haves
- Previous office nursing experience preferred
- Experience with electronic medical records preferred
- Spanish-speaking skills are a plus
Responsibilities
- Provide telephone triage and respond to patient calls
- Lab/diagnosis tracking
- Request/obtain medical records for new patients
- Coordinate and track patient referrals
- Complete medication prior authorizations as needed
- Assist other nursing staff as needed to support patient care
- Maintain accurate documentation in the EMR
