Our care teams are composed of the following: Nurses (RN & LPN) Certified Would Care Specialists Psychiatric Nurses Physical Therapists Occupational Therapists Speech Therapists Social Workers Certified Nursing Assistants (Home Health Aides) Utilizes the nursing process to provide clinical expertise, consultation, and direct patient care for people with or at risk for fecal and urinary diversions, acute and chronic wound, drains, tubes, fistulas and pressure ulcers. Responsible for developing and implementing a program to promote wound healing by means of educating and training staff, standardizing care, establishment of a comprehensive product formulary, and evaluation of outcomes in these areas. Functions in an educative role (staff in-services, patient teaching and community education), advocacy role, as well as performing research and administrative duties. Primary Duties Responsibility and accountability for assessment, planning, implementation and evaluation of ostomy and wound patients for patients on service with Evergreen Home Care Services. Functions as a consultant for staff and patients with ostomies, complex wounds, fistulas, ulcers and skin problems related to fecal and urinary incontinence .Responsibility and accountability for consultation, direct patient care, program development and education of fecal and urinary diversions or incontinence, and ostomies will depend on level of certification at ostomy and/or continence level.
