Greenfield Rehabilitation Agency is seeking a motivated and highly engaged Senior Community Coordinator (OT or PT) to support our senior living community in Walworth, WI and the surrounding Walworth County communities. This unique role blends direct patient care, leadership influence, and community relationship-building, making it an ideal opportunity for a therapist who enjoys autonomy, collaboration, and impact within senior care settings. Position Overview In this role, you will: Provide individualized occupational or physical therapy services to aging adults within assigned senior communities Support, mentor, and collaborate with therapy teams across multiple sites Serve as a primary point of contact for facility administrators and community partners Strengthen relationships and promote Greenfield Rehab’s mission and values within each community Work closely with the Regional Director to ensure clinical excellence, team satisfaction, and smooth daily operations This is a high-impact, hybrid clinical/leadership role ideal for an OT or PT who enjoys communication, relationship-building, and helping create strong, supportive care environments for seniors.

Requirements

  • Licensed Occupational Therapist (OT) or Physical Therapist (PT) in the State of Wisconsin
  • Experience in senior care settings (Assisted Living, Skilled Nursing, Memory Care, or Home Health) strongly preferred
  • Strong communication, leadership, and organizational skills
  • Ability to support multiple communities and foster positive relationships
  • Minimum of two years of experience preferred (strong candidates with less experience will be considered)
  • Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
  • Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.
  • Graduate of an accredited school with a minimum of a bachelor’s degree in Occupational Therapy.
  • Certification or license as required by the State of Wisconsin or applicable state to practice as an Occupational Therapist.

Nice To Haves

  • Experience in senior care settings (Assisted Living, Skilled Nursing, Memory Care, or Home Health) strongly preferred
  • Minimum of two years of experience preferred (strong candidates with less experience will be considered)
