Cleveland Clinic’s Social Work team provides caring, patient and family-focused social work services to meet basic human needs, enable clients to reach their optimal level of psychosocial functioning and enhance well-being. In this role, you will coordinate the transition of care to help patients move through the hospital and ensure that they have everything they need to make the transition home. Here, you will experience various educational opportunities and the ability to earn certifications within the field. A caregiver in this position works days from 7:00AM – 3:30PM, including rotating weekends and holidays.

Requirements

  • Bachelor's Degree in Social Work (BSW) from an accredited school of Social Work
  • Licensed Social Worker (LSW) in the state of Ohio
  • Basic Life Support (BLS) certification through American Heart Association (AHA) or American Red Cross
  • Knowledge of and practice in accordance with the NASW Code of Ethics
  • Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes
  • Requires normal or corrected hearing and vision to normal range.
  • Ability to perform work in a stationary position for extended periods.
  • Ability to communicate and exchange accurate information
  • Ability to work with physical records, such as retrieving and filing.
  • Ability to operate a computer and other office equipment.
  • Ability to travel throughout the hospital system.
  • Requires normal or corrected hearing and vision to normal range.
  • In some locations ability to lift up to 10 pounds

Nice To Haves

  • Three years of experience in healthcare systems to include exposure to and experience in pre-acute and post-acute care
  • Social Worker or Case Management Certification (CCM) within one year of eligibility
  • Hospital social work experience
  • Knowledge of community resources

Responsibilities

  • Assist patients and families in achieving improved health outcomes by reducing the bio psychosocial spiritual obstacles that interfere with their ability to use health resources effectively.
  • Assess patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
  • Assess patient and family's financial, social, emotional, educational and environmental resources and identify needs based on patient's current medical condition.
  • Develop an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patient's medical condition.
  • Participate in discharge planning activities for complex patients to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.
