SOCIAL WORKER / COUNSELOR
About The Position
The PRN Social Worker provides as‑needed clinical support to patients, families, and the interdisciplinary treatment team at Destiny Springs Healthcare. This role is responsible for delivering high‑quality psychosocial assessments, discharge planning, crisis intervention, and therapeutic support to promote safe and effective patient care. The PRN Social Worker contributes to a collaborative, patient‑centered environment and ensures continuity of services during periods of increased census, staff coverage needs, or emergent clinical situations. Position Summary: The social worker/ counselor is responsible for ensuring that the medically related emotional and social needs of the patients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines, and regulations.
Requirements
- Master’s degree in Social Work or related field.
- CPR certification, Basic Life Support, Behavioral Management Training required prior to patient interaction.
- Two years of related experience.
Nice To Haves
- Certification or licensure by Arizona State Board of Behavioral Health Examiners preferred.
Responsibilities
- Provides individual, group, and family therapy for adolescent and adult clients, reflective of treatment plan goals utilizing evidence-based practices.
- Develops a comprehensive social history and psychosocial assessment that includes the resident's problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.
- Orients patients and families to the facility, its services, its service limitations and client's rights.
- Involves patients (and families) in treatment planning by identifying measurable goals.
- Monitoring of treatment plan with the patient on a regular basis.
- Attends scheduled treatment team meetings and identifies and interprets psychosocial needs of residents for inclusion in the treatment plan.
- Completes mandatory documentation within timelines established by applicable regulations.
- Mediates issues that arise among patients, families, and staff.
- Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals and identifying unmet needs (e.g., transportation, outpatient treatment referrals, etc.)
- Provides patients with crisis management services.
- Develops a comprehensive discharge plan beginning upon admission in collaboration with patients, patient’s families, multidisciplinary staff, referral agencies, and outpatient services.
- Conducts process and psychoeducational group programming curriculum to include stress management, coping skills, etc.
- Conform with and abide by all regulations, policies, work procedures and instructions.
