Requirements

  • Ability to stoop, kneel, crouch, crawl, reach, stand, walk, push, pull, lift, grasp, and be able to perceive the attributes of objects such as size, shape, temperature, and/or texture by touching with skin, particularly that of the fingertips.
  • Ability to express and exchange ideas via spoken work during activities in which they must convey detail or important spoken instructions to others accurately, sometimes quickly and loudly.
  • Ability to express and exchange ideas via written assignments effectively and accurately.
  • Hearing to perceive the nature of sound with no less than 40 db loss @ Hz, 1000 Hz, and 2000 Hz with or without correction; ability to perceive detailed information through oral communication and to make fine discriminations in sound.
  • Performs repetitive motions with wrists, hands, and fingers.
  • Master’s Degree in Social Work or related field.
  • CPR certification and CPI/BLS are required prior to patient interaction.
  • Two years of related experience.

  • Licensed with Arizona Board of Behavioral Health preferred

Responsibilities

  • Develops a comprehensive social history and psychosocial assessment that includes the resident's problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.
  • Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals and identifying unmet needs (e.g., transportation, outpatient treatment referrals, etc.)
  • Provides patients with crisis management services.
  • Develops a comprehensive discharge plan beginning upon admission in collaboration with patients, patient’s families, multidisciplinary staff, referral agencies, and outpatient services.
  • Conducts process and psychoeducational and or process groups programming curriculum to include stress management, coping skills, Mindability-Roadmap to Peace of Mind, etc.
  • Attends scheduled treatment team meetings and identifies and interprets psychosocial needs of residents for inclusion in the treatment plan.
  • Completes mandatory documentation within timelines established by applicable regulations.
  • Mediates issues that arise among patients, families, and staff.
  • Orients patients and families to the facility, its services, its service limitations and residents' rights.
  • Involves patients (and families) in treatment planning by identifying measurable goals.
  • Visibility and accessibility on assigned units while maintaining appropriate boundaries.
  • Conform with and abide by all regulations, policies, work procedures and instructions.
  • Monitoring of treatment plan with the patient on a daily basis.
