About The Position

Work in this class involves providing intensive social work services in serious and complicated cases which require the flexible use of a wide range of social work skills and intervention techniques. Services are provided in a variety of settings, including neuro-medical centers, child care facilities, Head Start/Early Head Start programs, home environments, correctional facilities, juvenile justice facilities, and state institutions. Clients/patients have specialized dementia, mental health, substance abuse and/or developmental disability issues. Work may involve providing in-depth assessment of individuals and family dynamics and needs, assessment, intervention and treatment of patients/clients and or families. Work may involve training and modeling to direct care staff or families. Employees work with patients/clients and families to develop treatment plans and discharge plans into the community. The discharge planning involves coordinating with a variety of community based service providers. This is a full-time position (40 hours per week) with State Benefits.

Requirements

• Master’s degree in social work from an appropriately accredited institution; or
• Bachelor’s degree in social work from an appropriately accredited institution and one year of directly related experience; or
• Master’s degree in a human services field from an appropriately accredited institution and one year of directly related experience; or
• Bachelor’s degree in a human services field from an appropriately accredited institution and two years of directly related experience; or
• an equivalent combination of education and experience.

Responsibilities

• Evaluate offender’s situation and her ability to deal with it, developing a social history, psychosocial assessment, service plan and/or treatment plan and follow-up.
• Develop community or service resources.
• Perform assessments to identify needs and establish an appropriate service/treatment plan to provide supportive/directive counseling to inmates and families in addressing needs.
• Utilize knowledge of a range of services and of supportive level counseling and other counseling techniques to provide services.
• Understand social work practices and theories, an understanding of the availability and interactions of a variety of community services and agencies and of a range of service providers.
• Utilize agency guidelines, specific state or federal standards, and principles of a variety of supportive counseling techniques to provide services.
• Referrals are documented in accordance with agency requirements.
• Work with inmates, families, habilitation/treatment teams in the development and implementation of treatment plans.
• Contact with other professionals is required in the coordination of services, programs and consultation.
