LOCATION: HOUSTON, TX – NORTHWEST CORRIDOR (HWY 249) Our clinic is conveniently located at 17200 State Hwy 249, Suite 150, Houston, TX 77064, with easy access to Hwy 249, I-45, I-10, the Sam Houston Tollway, Hwy 290, and the Grand Parkway (99). W-2 Employee – Guaranteed Hourly Rate $33–$41/hour (based on experience) Guaranteed hours and benefits Minimum 20 hours/week for benefits eligibility 2. W-2 Employee – Pay Per Visit (PPV) $50–$76/hour (paid per patient visit) Benefits available at 20+ hours/week Higher earning potential with flexible scheduling 3.
