Requirements

  • Active, unencumbered Massachusetts RN licensure
  • Minimum two (2) years RN experience preferred; sub-acute, rehab, or skilled nursing experience
  • PointClickCare (PCC) experience
  • CPR certification
  • Demonstrate physical ability to stand, sit, walk, bend, squat, and lift/carry up to 20 pounds unassisted or 50 pounds with assistance
  • Proficient in reading, writing, and following instructions in English

Responsibilities

  • Provide skilled nursing care to sub-acute rehabilitation patients, including assessments, care planning, medication administration, and treatments
  • Monitor patient conditions, recognize changes in status, and communicate promptly with physicians and the interdisciplinary rehab team
  • Collaborate closely with PT, OT, ST, Case Management, and Social Services to support rehab goals and safe discharge planning
  • Supervise and delegate nursing tasks to LPNs and CNAs in accordance with Massachusetts scope-of-practice regulations
  • Accurately document assessments, treatments, and patient responses in the electronic medical record
  • Ensure compliance with CMS regulations, Massachusetts DPH standards, infection control, safety protocols, and patient rights
