About The Position

Responsible for providing support services to individuals living with a severe & persistent mental illness residing in independent apartments.

Requirements

  • Associated Degree in Human Services Field
  • High School Diploma, GED and at least two years of experience working in the Human Services field.
  • Ability to thrive in an independent, self-guided manner.
  • Understanding of mental illness and co-occurring disorders, engagement strategies, wellness self-management,
  • Working knowledge of services and community resources that will help the client to remain in stable housing.
  • Aptitude for developing collaborative relationships with community partners.
  • Empathetic individual with the ability to demonstrate poise, tact, and diplomacy.
  • Strong oral and written communication skills
  • Basic computer software knowledge (i.e., Word, Excel, Microsoft Outlook etc.…).
  • Valid driver’s license, acceptable driving record with auto insurance coverage as defined by agencies carrier.

Responsibilities

  • Assist clients to obtain safe and affordable housing in the Lamphear complex.
  • Assist clients in overcoming the reasons for homelessness and to gain self-sufficiency.
  • Provide education to reduce risk of homelessness and maintain stable, permanent housing.
  • Conduct home visits with clients at least once monthly, increased frequency if needed, to review plan and goal progress.
  • Assist clients in applying for mainstream benefits (including health care, social services, mental health, substance abuse services and linkage to affordable Health Care Act), for which they are eligible.
  • Assist clients in accessing employment and educational services.
  • Provide on-the-job support as needed to ensure client is fully prepared to maintain independent employment.
  • Assistance in understanding the terms of a lease and the responsibilities of being a tenant.
  • Create a service plan and identify S.M.A.R.T goals to assist client in moving towards full independence.
  • Assistance in learning and applying daily living skills such as time management, nutrition, budgeting, and hygiene.
  • Assistance with budgeting and money-management needs
  • Assistance in learning and using public transportation.
  • Assistance in learning the locations of service providers and how to access them.
  • Assistance with learning and using shopping skills.
  • Collaborate with community partners to ensure clients have full access to all services they want and need!
  • Maintain appropriate documentation required by ESSHI.
  • Complete required trainings within six months of hire (Trauma Informed Care, Motivational Interviewing, Mental Health First Aid, Professional Boundaries, and others and deemed appropriate).
  • Report to work on time and work your scheduled shift, except in cases of emergency.
  • In the case of an emergency, contact your immediate supervisor and/or on call designee for guidance.
  • Complete, clear, quality, integrity based and timely documentation for each client on your assigned caseload.
  • Effectively discharge the key job functions of your job in accordance with Supportive Services rules and regulations, AHD Code of Conduct and Standard of Acceptable Behavior
  • Support a Non-Discrimination/Anti-Harassment/Sexual Harassment free work environment, foster a respectful environment and refrain from unprofessional conduct as referenced in the Personnel Policy Manual
