About The Position

The Targeted Case Manager (TCM) provides office-based case management services to individuals diagnosed with Serious Mental Illness (SMI) who are not enrolled in Assertive Community Treatment (ACT). Services focus on assessment, service coordination, linkage, monitoring, advocacy, and ongoing support.

Requirements

High school diploma or GED required; Associate's degree in human services, healthcare administration, or a related field preferred.
Prior experience in a healthcare, behavioral health, or human services setting strongly preferred.
Experience working with individuals with Serious Mental Illness preferred
Knowledge of community resources, benefits systems, housing, and social services
Proficiency in Microsoft Office Suite and ability to learn new software (e.g., EHRs, scheduling systems).
Strong attention to detail and organizational skills to support accurate recordkeeping and data entry.
Comfort using phones, printers, and standard office equipment.
Warm, clear, and professional communication style across all interactions.
Ability to maintain confidentiality, emotional regulation, and professionalism in high-pressure situations.
Friendly, responsive demeanor with clients experiencing mental health or substance use challenges.
Ability to triage client needs and connect them to the appropriate person or resource.
Strong prioritization and decision-making skills in a fast-paced environment.
Willingness to learn, receive feedback, and adapt to changing systems.

Nice To Haves

Bilingual in English and Spanish or another commonly spoken language is a plus.

Responsibilities

Provide comprehensive, ongoing Targeted Case Management services to individuals diagnosed with SMI who are not enrolled in ACT.
Assess client needs across multiple life domains, including medical, behavioral health, housing, employment, legal, financial, and social supports.
Collaborate with clients and their treatment teams to support development, implementation, and coordination of individualized treatment plans.
Assist clients in staying engaged with services, including support with attending appointments, accessing recommended services, and following through with care recommendations.
Support clients in maintaining benefits and coverage, including insurance, Medicaid eligibility, and other entitlements, by assisting with applications, renewals, and follow-up as needed.
Provide referrals and linkage to appropriate internal and community-based services and resources.
Monitor service engagement, follow up on referrals, and support treatment teams in revising plans based on client progress and changing needs.
Advocate for clients to reduce barriers and ensure access to necessary services and supports.
Assist clients in navigating complex systems such as benefits, housing, healthcare, and legal systems.
Conduct ACT appropriateness screenings at intake and during treatment plan reviews.
Notify the ACT Team Lead and Outpatient Director when clients meet criteria for ACT services.
Provide interim Targeted Case Management services while ACT services are being developed or implemented, as appropriate.
Coordinate care with internal and external providers to support a holistic, integrated approach to services.
Ensure follow-up and support to increase the likelihood that external referrals and appointments are attended.
Maintain communication with primary care providers, behavioral health providers, social service agencies, housing authorities, legal advocates, and educational or vocational programs.
Be cross-trained to function as a Care Coordinator, assist with referral processes, community support and support crisis response activities in accordance with agency protocols and scope of practice.
Document all services in The Well’s EHR in accordance with CCBHC and organizational standards.
Deliver services in-person, by phone, or via telehealth based on client need, preference, and medical necessity.
Adjust frequency and intensity of services based on level of functioning, medical necessity, and treatment goals.
Conduct drug/alcohol screening activities for the contingency management program.
Manage outpatient concessions, including handling cash transactions.
Provide backup support to other administrative staff as needed.
Participate in staff meetings, trainings, and required compliance sessions.
Perform other related duties as assigned.
