About The Position

A vital and compassionate Home Health Team Assistant is needed to provide essential support to clients and the clinical team. This role suits someone who is detail-oriented, empathetic, and enjoys making a tangible difference in people's lives. The Home Health Team Assistant will ensure smooth and coordinated care for clients in their residences, working closely with nurses, therapists, and other healthcare professionals.

Requirements

• Communication skills: Strong written and verbal communication abilities are crucial for interacting with team members, clients, and other stakeholders.
• Attention to detail: The capacity to notice even the smallest details and maintain accuracy in documentation, scheduling, and all tasks.
• Proactivity and Adaptability: The willingness to anticipate needs and find solutions before problems arise, as well as the ability to adjust to changing priorities and situations without lowering work quality.

Responsibilities

• Communication Hub: Acting as a central point of contact for internal and external inquiries, managing emails and correspondence, and liaising with stakeholders to ensure effective communication channels.
• Administrative Support: Handling various administrative tasks like document preparation (word processing, spreadsheets, presentations), editing and formatting documents, filing, photocopying, scanning, data entry, and organizing office supplies
• Team Support: Assist with project-related tasks, such as tracking deadlines, gathering information, and making phone calls to patients
