About The Position

Are you ready to transition from direct clinical work into a leadership role? As a Team Lead at Telecare’s AgeWise program, you will mentor a multidisciplinary team dedicated to helping older adults (59+) rediscover their health, hopes, and dreams. This is an entry-level management position designed for clinicians who want to sharpen their supervisory skills while remaining deeply rooted in community-based care. AgeWise is a community-based program serving older adults. AgeWise serves a maximum of 600 individuals, aged 59+. Agewise's Strengths-Based Case Management is a consumer-driven and highly collaborative program. Teams work with members to identify needs and build a treatment plan that is collaborative and comprehensive. The goal is to assist people living in the community and ensure members are linked to housing, medical care, psychiatric care and natural supports. Agewise's Institutional Case Management (ICM) serves members who are conserved and placed in a long-term care facility. The program provides members with support in obtaining and maintaining benefits, care coordination, discharge planning, and advocacy. Members are assessed to ensure that they are placed in the least restrictive environment possible.

Requirements

  • Master’s Degree and one (1) year of experience in mental health setting
  • One (1) year supervisory experience required or supervisory training within six (6) months of employment
  • Licensed eligible or waivered mental health professional as confirmed by State regulatory/licensing board (e.g., ASW, IMFT, or PCCI)
  • Sensitivity to multi-cultural populations and issues
  • Must be at least 18 years of age
  • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
  • All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver’s license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.  (weekly mileage reimbursement at the IRS rate)

Responsibilities

  • Supervise a passionate team of providers, ensuring they have the support and resources needed to deliver excellent care.
  • Act as the bridge between multidisciplinary teams and outside agencies to ensure no member falls through the cracks.
  • Work within our Strengths-Based Case Management model to help seniors navigate housing, medical care, and psychiatric support so they can live independently and with dignity.
