About The Position

At Sully.ai, We’re Building the Most Impactful Healthcare Company on Earth We believe that access to a great doctor is a basic human right. Today, that’s not a reality. Delays, misdiagnoses, administrative chaos, and burnout plague the system. Our Mission: One Human, One Doctor. We build AI teammates that augment clinicians — scribes, nurses, receptionists, translators — all powered by our own world-class models and deployed in real-world care. About Sully Sully is building the AutonomousOS for healthcare, covering the end to end doctor patient journey. We work with health systems, specialty practices, and enterprise clinics across the US and internationally. We've completed more than 2 Million patient interactions and completed over 5 millions clinical tasks to date. We move fast, we ship to production, and we hold a high engineering bar. This is an early-stage founding team role, not a scaled org hire. About the Role This is a full ownership, hands on technical role. You own a small, exceptional team and take end-to-end ownership of a core product domain - from architecture decisions to shipping to on-call. You will manage 2–3 engineers today and grow the team as the domain scales. You are not inheriting a process. You are defining one. The engineers who join Sully in this window will shape how the engineering org thinks, builds, and operates for the next decade.

Requirements

  • You have built things that mattered: as a founder, founding engineer, or first technical hire at a company that shipped real product to real users
  • You have owned backend systems under production load: not just contributed to them
  • This is not a manager role that has drifted away from the keyboard. You are a strong programmer. You write code every day.
  • You are ready to lead a small team and grow into owning an engineering org: you have led people in some capacity (even informally) and made them better
  • You communicate with high clarity: in PRs, OKRs, architecture docs, and direct conversation
  • You thrive in ambiguity: you define the problem before you solve it, and you do not need a perfect spec to start

Nice To Haves

  • Experience in AI SaaS, former Co-Founder/Founder
  • Hands-on work with LLMs, ASR, or real-time audio/NLP pipelines
  • Comfort building at the infrastructure-product boundary (not just pure backend or pure ML)
  • Previous founding engineer or early employee experience (sub-20 headcount)

Responsibilities

  • End-to-end ownership of a core product domain: architecture, implementation, and reliability
  • Technical direction: make meaningful architectural calls, document them, and defend them
  • Quality bar: code review standards, testing discipline, and engineering norms for your team
  • Shipping velocity: keep a small team unblocked, focused, and moving at a pace that compounds
  • Team growth: hire 3-5 engineers under you over the next 12 months and make them better
  • Cross-functional partnership: work directly with product, research, and clinical teams — no layers
