About The Position

Progressive Rehab of PA, Inc., is looking to hire a full-time Vocational Case Manager in Milford, PA. Our mission is to empower people with acquired brain injuries to live their best lives. Progressive Rehab provides services to approximately 70 clients. This Case Manager would work with our more independent clients. The Vocational Case Manager is responsible for providing comprehensive vocational counseling and case management services to clients served by the program. This role supports participants in identifying career interests, developing employment skills, and achieving vocational goals while coordinating services, resources, and supports that promote independence and successful community reintegration.

Requirements

  • Bachelor’s or Master’s Degree in Human Services or a related field.
  • Valid driver’s license with a minimum of a five-year clean driving record and the ability to travel for community-based work, employer visits, and job coaching as needed.

Nice To Haves

  • Vocational Counseling specialty and/or relevant experience preferred.
  • Experience working with individuals with Traumatic Brain Injury (TBI) or neurological injury strongly preferred.
  • Knowledge of vocational rehabilitation practices, employment law, ADA accommodations, and community support resources.

Responsibilities

  • Develop individualized vocational plans aligned with rehabilitation goals and participant abilities.
  • Provide job readiness training to clients.
  • Assist clients in working with the Office of Vocational Rehabilitation (OVR), including navigating application and approval processes.
  • Support clients in identifying and securing appropriate employment opportunities through employer outreach and community partnerships.
  • Provide on-site job coaching and ongoing support to promote job retention and skill adaptation.
  • Advocate for workplace accommodations and injury-related needs to support successful employment integration.
