About The Position

At Lucet, we are industry leaders in delivering comprehensive, integrated health care solutions designed to help individuals lead healthy, balanced lives. Our unique combination of people, clinical expertise and technology is changing how health plans manage their networks, providers manage care pathways, and members manage their health with the right support at the right time. By connecting people to the appropriate behavioral, clinical and physical health services, we improve outcomes and overall well-being. Our mission and our passion is to improve physical, behavioral and social health for those we serve. Combining our expertise in behavioral health with in-home medical care services, Lucet provides whole health solutions that address chronic conditions and other barriers to wellness. Our collaborative care model includes behavioral health care navigation, proactive coordination with primary care providers, 24/7/365 behavioral health crisis support, comprehensive medication management and long-term care planning—all tailored to meet individual needs. We are seeking In‑Home Assessment Providers (Nurse Practitioners or Physician Assistants) to support our health plan partners by delivering high‑quality, in‑home preventive health assessments. In addition to this flexible fee‑based model, you’ll have the opportunity to support meaningful health outcomes, improve access to care, and work independently with the full support of a dedicated clinical team.
