About The Position

The Care Coordinator is an integral member of the interdisciplinary Case Management team for Astrana Health's Care at Home program. This role provides administrative and operational support to Case Managers by facilitating member outreach, coordinating referrals, managing documentation, and supporting care transitions. The Care Coordinator ensures timely processing of referrals, accurate documentation, and effective communication with members, providers, hospitals, and internal clinical staff while maintaining compliance with organizational policies and HIPAA regulations.

Requirements

  • High School Diploma or equivalent
  • Minimum of two (2) years of experience in a medical office, healthcare practice, managed care, IPA, or MSO environment
  • Knowledge of medical terminology
  • Proficiency in Microsoft Office Suite
  • Strong organizational and time management skills
  • Ability to multitask and prioritize in a fast-paced environment
  • Effective verbal and written communication skills
  • Ability to maintain confidentiality in accordance with HIPAA regulations

Nice To Haves

  • Experience as a Certified Medical Assistant (CMA), scheduler, or medical office front desk staff
  • Experience supporting Case Management or Care Coordination teams

Responsibilities

  • Provide day-to-day administrative support to Case Managers and the interdisciplinary team
  • Manage and assign incoming referrals (electronic systems, fax, portals, etc.) to appropriate team members
  • Manage and distribute incoming faxes and clinical documentation
  • Identify member admissions and discharges from acute care settings and notify appropriate clinical staff
  • Obtain and transmit discharge summaries and related documentation
  • Communicate with hospitals, skilled nursing facilities (SNFs), acute rehabilitation facilities, and other admitting facilities regarding admission status and discharge planning updates
  • Conduct telephonic outreach to members to support program engagement, appointment scheduling, reminders, follow-up calls, and satisfaction surveys
  • Provide coverage of department phone lines during regular business hours; triage and route calls appropriately
  • Escalate urgent clinical matters promptly to licensed clinical staff in accordance with department protocols
  • Create and maintain member cases and appointment information within EMR and care management platforms
  • Ensure documentation is accurate, timely, and compliant with departmental turnaround standards
  • Assist in researching and resolving operational issues within the department
  • Manage office supplies and support inventory needs for the CM team
  • Support printing and mailing needs for the interdisciplinary team
  • Establish and maintain organized filing systems and complete clerical functions to ensure efficient department operations
  • Cross-train on additional Coordinator responsibilities to provide coverage as needed
  • Adhere to all department policies and procedures, including attendance, timekeeping, dress code, and confidentiality standards
