About The Position
"Navitas Healthcare, LLC" is seeking a dedicated and experienced Case Worker to provide comprehensive social services to individuals and families in need. Under general supervision, the Case Worker will assess client needs, determine eligibility for public assistance programs, and develop and implement individualized service plans to promote client stability and well-being.
Requirements
- Minimum 3 years of experience in social services, case management, or a related field
- Strong interpersonal, communication, and counseling skills
- Ability to assess complex social situations and make sound, informed decisions
- Excellent organizational skills with strong attention to detail
- Ability to manage multiple cases independently
- Bachelor’s Degree from an accredited college or university (required)
Responsibilities
- Provide social services to individuals applying for or receiving public assistance, SNAP (food assistance), medical assistance, and protective services
- Assess client circumstances and determine eligibility for appropriate social service programs
- Develop, implement, and monitor individualized service plans
- Conduct interviews, counseling sessions, and investigatory activities related to client cases
- Coordinate services for adults requiring institutional care or protective services
- Maintain accurate, timely, and compliant documentation in accordance with regulatory standards
- Collaborate with internal teams, external agencies, and community organizations
- Ensure confidentiality and adherence to applicable laws, policies, and procedures
- Perform additional related duties as assigned
