The Health Information Management Clerk is responsible for retrieving and reconciling medical records and loose documentation from all patient care units. Assist with any clerical duties assigned by HIM Supervisor and Contributes to team effort by accomplishing related results as needed. ESSENTIAL FUNCTIONS INCLUDE BUT NOT LIMITED TO: Maintains medical records operations by following policies and procedures. Sustains Master Patient Index. Resolves medical record discrepancies by collecting and analyzing information. Retrieves discharged medical records from all patient care units on a daily basis. Responsible for the receipt, scanning, indexing of medical record documents into the hospital’s electronic health record (EHR) in a timely and accurate manner. Scans and indexes loose reports/documents that are identified as critical to coding and prioritizes them according to policy. Assist with (ROI) and the coordination of medical information to all requesters, ensuring the validity and authorization of the request. Assist with the processing of subpoenas and court orders. Maintains patient confidence and protects hospital operations by keeping information confidential; following release-of-information protocols. Assists in retrieving medical records for physician completion when necessary. Processes facility specific HIM mail as applicable. May assist with physician questions related to EMR and the patient portal or other issues that may arise. Responsible for the scanning of medical record documents into the hospital’s electronic health record (EHR). Conserves resources by using equipment and supplies as needed to accomplish job results. Assist with release of information (ROI) requests, answering of phones and processing of HIM mail. Other duties as assigned.
