Requirements

  • Current LPN license in the state of Connecticut.
  • Previous experience in home care, nursing, or a related field preferred.
  • Strong clinical and organizational skills.
  • Effective communication and interpersonal skills for working with diverse populations.
  • Ability to work independently and as part of a healthcare team.
  • Reliable transportation and a valid driver's license.
  • Current CPR certification is required.
  • Flexible schedule to accommodate part-time availability.

Responsibilities

  • Perform initial assessments of clients to determine their medical and caregiving needs.
  • Collaborate with RNs and other healthcare team members to develop comprehensive care plans.
  • Monitor and document patient status, ensuring all information is accurate and up to date.
  • Provide education and support to clients and families regarding health management and care strategies.
  • Ensure compliance with all healthcare regulations and company policies.
  • Communicate effectively with clients, families, and the healthcare team to promote seamless care transitions.
