About The Position

The Intake/ Home Care Coordinator serves as the first point of contact for the UnityPoint at Home Intake Department, playing a key role in connecting patients with home care services. In a fast- paced, call- center environment, this position manages incoming referrals via phone, fax, and electronic systems, reviews referral information to identify patient needs, accurately enters data into the EMR, and assists in coordinating admission visits. The coordinator collaborates regularly with hospitals, physicians, skilled nursing and board and care facilities, patients, families, and internal teams to ensure a smooth and timely transition to care. Requirements

  • Education: HS Diploma or equivalent with successful completed course work
  • Minimum Experience: Six months phone based customer service experience. Experience should include high volume, fast paced customer excellence environment such as a call center or similar phone based service role.

Responsibilities

  • Maintains a broad knowledge in all UnityPoint at Home service lines including, Home Health, Hospice and Infusion services.
  • Routinely monitors for new referrals via fax, EMR and other electronic applications.
  • Accepts all new referrals and initiates referral documentation in the EMR and ensures accurate information and documentation is included in patient record to facilitate the admissions process, which includes diagnosis codes after validating with RN.
  • Secures, confirms and accurately documents provider orders in a timely manner.
  • Operates computer system to record/document and retrieve information to ensure appropriate agency services are provided in a timely manner.
