About The Position

The Lead Case Manager provides clinical and administrative oversight to the Case Management team, ensuring the delivery of person-centered, trauma-informed support to shelter residents. This role serves as a subject matter expert in Medicaid billing requirements and HMIS documentation, guiding the team in developing effective service plans and achieving housing stability for residents.

Requirements

  • Bachelor's in Social Work, Counseling, or a related field (LCSW or LCPC preferred).
  • Minimum of 3 years of experience in social services, with at least 1 year in a supervisory or lead role.
  • Advanced knowledge of Medicaid billing, HMIS, and trauma-informed care.
  • Exceptional organizational, communication, analytical, and problem-solving skills.
  • Ability to work independently, prioritize tasks and meet deadlines
  • Participation as a team player and sensitive to issues of diversity.
  • Strong documentation skills, collaboration across shifts, and adaptability to resident needs.
  • Computer knowledge of MS Office products, and email systems.
  • Ability to work in a fast-paced shelter setting.
  • English proficiency required

Responsibilities

  • Supervise and mentor a team of Case Managers, providing guidance on complex cases and biopsychosocial assessments.
  • Facilitate regular clinical supervision and team meetings to review resident progress and identify service gaps.
  • Oversee the distribution of caseloads and ensure continuity of care during shift handoffs.
  • Support the hiring, onboarding, and professional development of new case management staff.
  • Serve as the primary auditor for case files, ensuring all documentation meets Medicaid case management standards and audit requirements.
  • Review progress notes to ensure they clearly reflect identified needs, interventions, and medical necessity.
  • Act as the point of contact for Medicaid-related documentation questions and service eligibility.
  • Ensure the team is proficient in HMIS and other required data systems.
  • Provide expert support for residents with high-acuity mental health or substance use barriers.
  • Lead the team in applying SMART goals to individualized action plans and Housing First principles.
  • Oversee crisis response protocols, ensuring staff use trauma-informed de-escalation approaches during emergencies.
  • Coordinate with internal leadership and external partners to enhance the network of housing and medical resources.
  • Must comply with agency requirements of strict client confidentiality, professional ethics and agency policies and procedures.
  • Must participate in the following but not limited to monthly staff meetings; training (travel often required); fundraising events; community fairs; client events, outreach efforts and other events as directed by leadership.
