About The Position
At Atlas Medical, we're growing-and we're looking for a people-first, relationship-driven professional to join our team as a Community Liaison / Patient Care Navigator (PCN). If you thrive on connection, love being out in the community, and want to make a real difference in healthcare-this role is for you. You'll play a key role in expanding access to our full continuum of care with Aegis Healthcare, including Mobile Primary Care, Home Health, Palliative, Hospice, Therapy, DME, and Medical Transport. Requirements
- Relationship-builder with 2+ years in marketing, outreach, or referral development
- Passionate about healthcare and making a difference
- Self-starter who thrives in a fast-paced, people-focused role
- Strong communicator (in person, over the phone, and in writing)
- Organized, adaptable, and goal-driven
- Valid driver's license + reliable transportation
- Auto insurance & registration
Nice To Haves
- Familiarity with the local senior care or healthcare space is a plus
Responsibilities
- Grow and manage referral partnerships across your territory
- Connect with patients, families, and providers to guide care solutions
- Represent Atlas at community events, presentations, and networking opportunities
- Identify new growth opportunities and drive strategic outreach
- Collaborate with internal teams to ensure seamless patient experiences
