About The Position

When a patient requires care from different facilities and/or practitioners they need a Patient Nurse Navigator to identify needs and remove any barriers to timely, appropriate care. Our Nurse Navigators guide the patient from diagnosis through treatment and act as the central point of contact for the patient and the entire medical team. It’s a job that requires a strong communicator with excellent organization skills. WHAT YOU CAN EXPECT Oncology Breast Program · Monday-Friday · Flexible shifts – 4-10’s or 5-8’s · Hours: 8:00 am -5:00 pm · No weekends, no holidays · This role involves interacting with approximately 10–15 patients per day, either by phone or in person. · Provide clinical resource guidance on where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information. · Assess patients for psychosocial distress and referral to a medical social worker or counselor if support that is more intensive is required. · Address tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more. Refers patients to the appropriate contact, such as a social worker, for support needed. · Function as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services through the continuum of care. · Empower patients, families, and caregivers to assume as much responsibility for their care as possible, within the constraints of age, illness, and other factors. · Educate patients about their disease, prescribed treatment, and side effects of treatment and how to manage them.

Requirements

  • Required Associate’s Degree Nursing/Patient Care
  • Registered Nurse (RN) - State Licensing Board
  • Basic Life Support Program (BLS) - American Heart Association

Nice To Haves

  • Preferred Bachelor’s Degree Nursing / Patient Care
  • 1-year related experience Preferred
