JOB SUMMARY: Coordinates the post-acute plan discharge arrangements including post-acute placement and Durable Medical Equipment (DME) set up. Provides logistical support to effect a safe and timely discharge for patients to include but is not limited to patient discharge disposition and arrangements, post-acute discharge planning, and coordination of discharge activities.

Requirements

  • High School diploma or equivalent.
  • One year healthcare experience in an acute care setting preferred.
  • Ability to read, understand, and follow through on written instructions.
  • Knowledge of multiple software programs, including word-processing, spreadsheet, and Internet.
  • Familiar with office machines and equipment, such as copier and fax machine.
  • Able to read medical terminology and assemble results from medical record.
  • Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
  • Highly organized and efficient in establishing priorities and performing tasks within specific timeframes.
  • Self-motivated requiring minimal supervision.
  • Must possess good physical health.
  • Some requirements include but are not limited to standing, sitting or walking for long periods of time.
  • Lifting at least 20 pounds is required.

Nice To Haves

  • LPN, Social Worker, or Medical Assistant experience preferred, with current Louisiana license or certification based on their credentials.
