About The Position

We are seeking an experienced Registered Nurse (RN) for a Home Care role serving adult patients in a community setting. This position involves traveling to patients’ homes to provide comprehensive, patient-centered care and support recovery and wellness.

Requirements

• Active New York State RN License (Primary Source Verified)
• Minimum 2 years of recent Med/Surg experience (within the last year)
• Prior home care experience (required)
• Basic Life Support (BLS) certification from AHA
• Strong patient assessment and clinical decision-making skills
• Proficiency in IV therapy and wound care
• Ability to work independently in a home care environment
• Excellent communication and patient education skills
• Must be comfortable traveling to patient homes
• Must be available to work every other weekend

Nice To Haves

• Advanced Cardiovascular Life Support (ACLS) (required for certain regions)

Responsibilities

• Assess, plan, implement, and evaluate individualized patient care plans
• Administer medications and monitor patient responses
• Perform wound care, IV therapy, and post-operative care
• Monitor vital signs and report any changes in patient condition
• Educate patients and families on treatment plans and recovery processes
• Maintain accurate and timely documentation
• Collaborate with healthcare teams on care coordination and discharge planning
