About The Position

The Registered Nurse – Population Health is responsible for delivering patient-centered care management services to a panel of high-risk, chronically ill, or recently hospitalized patients. The RN works across the care continuum to proactively improve health outcomes, reduce avoidable utilization, and ensure effective care coordination. Using clinical expertise, Epic-based tools, and established workflows, the RN provides chronic care management, transitional care, and risk reduction services. This role focuses on coordinating team-based care and implementing tracking systems to detect and manage chronic conditions early. Additionally, the RN collaborates with patients, families/caregivers, internal and external resources, and providers to support the clinic's objectives and ensure optimal care for the chronically ill patient population.

Requirements

  • Maintain nursing licensure, in good standing, acceptable in the State of Washington.
  • Active BLS (basic life support) certification.
  • Familiarity with Epic.
  • Customer Service.
  • Experience working in a healthcare setting.
  • Demonstrates the ability to perform the required technical skills for a Registered Nurse.
  • Demonstrates the ability to perform the required technical skills for specific clinic.
  • Ability to learn and adapt to technology.
  • Initiates and participates in activities that enhance daily operations of the clinic and organization.
  • Ability to manage time, tasks, and resources appropriately
  • Excellent verbal and written communication skills with colleagues, patients, and customers.
  • Strong typing skills.
  • Ability to read and write clearly, and understand policies, labels, and written instructions.
