About The Position

Social Services is now accepting applications to be considered for a Full-Time Social Worker in the Home and Community-Based Services Unit. Information on Clay County’s insurance benefits can be found here: Fringe-Benefit-Summary_2026 Status: Exempt MINIMUM REQUIREMENTS Bachelor’s degree from an accredited four-year college or university with a major in Social Work, Psychology, Sociology or closely related field. 1-3 years prior work-related experience Valid driver’s license Must be a current Clay County Social Worker LSW Preferred Successfully complete a background investigation TO BE CONSIDERED FOR THE POSITION, SUBMIT THESE MATERIALS Clay County Application must be submitted for all County positions. All veterans who are certified will be considered for appointment. DUTIES AND ESSENTIAL FUNCTIONS The listed examples may not include all the duties performed by all positions in this class. Provide case management and support coordination for individuals with developmental disabilities or related conditions under Rule 185 and home- and community-based service programs. Assess individual and family needs, develop and implement service plans, and coordinate services to promote independence and community living. Collaborate with interdisciplinary teams, including health care providers, schools, and community resources, to ensure integrated and person-centered care. Monitor and evaluate ongoing service effectiveness and client progress, ensuring compliance with state and federal program requirements. Maintain accurate and timely documentation in accordance with agency and state guidelines. Participate in multidisciplinary team meetings and county/state trainings as required. Other duties as assigned.

Requirements

  • Bachelor’s degree from an accredited four-year college or university with a major in Social Work, Psychology, Sociology or closely related field.
